The association between serum 25(OH)D levels and insulin resistance in Korean adults.

Abstract
Background : Vitamin D is important for muscle and bone health. However, its many physiologic actions outside the musculoskeletal system are now being explored. Some but not all observational studies have shown an inverse relationship between vitamin D levels and glucose metabolism. The aim of this study was to investigate the association between serum 25-hydroxy vitamin D(25(OH)D) levels and insulin resistance in Korean adults who were not taking diabetic medications.

Methods : This cross-sectional study included 6,710 adults aged 19 years and over who visited a health promotion center. Physical examinations and laboratory tests including those for serum 25(OH)D, fasting glucose and insulin levels were performed. Multivariate linear regression analysis was performed to identify the major predictors of high homeostatic model assessment-of insulin resistance (HOMA-IR) values. Logistic regression analyses were also used to estimate the odds ratios (ORs) for high HOMA-IR values (¡Ã2).

Results : The results showed that serum 25(OH)D levels influenced the HOMA-IR after adjusting for age, gender, body mass index, lifestyle factors, and all other variables. Low vitamin D levels (25(OH)D <10 ng/mL and 10ng/mL ¡Â25(OH)D <20 ng/mL) were associated with significantly increased odds of high HOMA-IR compared with those with sufficient vitamin D levels (25(OH)D >30 ng/mL) (OR=1.76, 95% confidence interval(CI)=1.18?2.62 and OR=1.50, 95% CI=1.03?2.16, respectively).

Conclusion : Serum 25(OH)D levels were negatively associated with high HOMA-IR values in adults who were not taking diabetic medications. This finding suggests that maintaining sufficient vitamin D levels might be important for prevention of insulin resistance.
