Background: Smoking is an important health problem. Therefore, we examined the association between smoking and health-related quality of life (HRQoL) among Korean adults.
Methods: We analyzed the data of 17,871 adults aged 19 years or older who participated in the fifth Korea National Health and Nutrition Examination Survey (2010-2012). The subjects were classified as nonsmokers, ex-smokers, and current smokers. HRQoL was measured by using the EuroQol-5 dimension (EQ-5D) and EQ-visual analogue scale (VAS) scores. Impaired HRQoL was defined as the EQ-5D index and EQ-VAS scores in the lowest 20% range and some or extreme problems in the EQ-5D domains. Logistic regression analysis was used to calculate the odds ratio (OR) and the 95% confidence interval (CI) for the effects of smoking status on impaired HRQoL.
Results: The adjusted ORs for impaired HRQoL for current smokers compared to nonsmokers were higher for the EQ-5D index and EQ-VAS scores (OR, 1.17; 95% CI, 1.17 to 1.17 and OR, 1.37; 95% CI, 1.36 to 1.37, respectively). In addition, the adjusted ORs for impaired HRQoL for current smokers compared to ex-smokers were higher for the EQ-5D index and EQ-VAS scores (OR, 1.21; 95% CI, 1.21 to 1.21 and OR, 1.23; 95% CI, 1.23 to 1.23, respectively). Current smokers were found to have more problems within all domains of the EQ-5D, compared to nonsmokers and ex-smokers.
Conclusion: Smoking is associated with lower HRQoL among Korean adults, compared to ex-smokers and nonsmokers.
