The Efficacy of Ultrasonography and Computerized Tomography for the Diagnosis of Acute Appendicitis

Abstract
Background: The aim of this study was to compare the accuracy of computed tomography (CT) and ultrasonography (US) for diagnosis of acute appendicitis.

Methods: We retrospectively the sensitivity, specificity, positive predictive value, and negative predictive value obtained from 220 patients who were suspected of having appendicitis and underwent CT or US between May 2008 and June 2010.

Results: One hundred and ninety-five patients had acute appendicitis at final diagnosis. Analysis of the CT and US images revealed a sensitivity of 99.47% and 95.62%, respectively, and a specificity of 87.50% and 81.81%,respectively. The positive predictive values were 97.9% and 95.26%, respectively, while the negative predictive values were 72.41% and 53.33%, respectively.

Conclusion: The CT imaging results were more accurate than those of US for diagnosis and exclusion of acute appendicitis. Hence, we conclude that the use of CT can substantially decrease unnecessary laparotomies and postoperative complications.
