Validity of Morisky Medication Adherence Scales in Assessment of Statin Adherence in Dyslipidemic Patients

Abstract
Background : Statins are potent drugs for lowering the levels of low density lipoprotein cholesterol (LDLC) and thus, preventing cardiovascular diseases. However, their effectiveness is significantly compromised by inadequate dose, inadequate response, failure to make life style modifications, and medication non-adherence. With respect to poor adherence to medication, clinicians have to identify the adherence problem in a clinically relevant manner.

Methods : Seventy patients who were selected randomly completed the Morisky Medication Adherence Scale (MMAS) questionnaire. Four patients undergoing secondary prevention were excluded. Sixty patients were divided into two groups based on whether they achieved the LDLC goal. This prospective study examined the relationship between MMAS scores and LDLC goal achievement.

Results : The average age of the selected patients was 64.57 years, and 84.8% of them were men. Forty patients achieved their LDLC goal. T-test revealed that the patients who achieved the LDLC goal had significantly lower LDLC levels (79.03¡¾17.45 mg/dL vs. 118.81¡¾25.57 mg/dL, P<0.001) and higher MMAS scores (3.43¡¾0.636 points vs. 2.42¡¾0.785 points, P<0.001) than the patients who did not achieve their LDLC goal. Additionally, as per the results of the chi-square test, the goal achievers were more adherent than the non-achievers (odds ratio, 1.35; 95% confidence interval, 107.67-129.95; P=0.002).

Conclusion : Medication non-adherence may be responsible for failure to achieve the LDLC goal in many patients who are prescribed statins. In routine clinical care, the structured MMAS questionnaire may prove to be an effective tool for clinicians to assess medication non-adherence.
