Usefulness of Esophagogastroduodenoscopy according to Age for Management Suspected Fish Bone Impaction in Patients

Abstract
Background: A swallowed fish bone can, though rarely, cause serious complications in the esophagus. Thus, the usefulness of esophagogastroduodenoscopy for detecting possible complications should be evaluated according to patient age.

Methods: In this study, 1,809 patients who were admitted to the emergency, internal medicine, pediatrics, and ear, nose and throat departments of a hospital in Busan were enrolled. Data regarding patient sex and age, the position of the ingested fish bone, the time of arrival at the hospital after fish bone ingestion, and the removal procedures performed in the various departments were obtained from the medical records of the patients and analyzed retrospectively.

Results: Among the 1,809 patients who swallowed a fish bone, 924 were women. The age difference between the sexes was statistically significant (P<0.01). Fish bones were detected in 1,080 patients. In those younger than 19 years, the most common affected site of the ingested fish bones was the oropharynx. Meanwhile, in those older than 70 years, the larynx was the most affected site (P<0.05). The detection rate by esophagogastroduodenoscopy was 14.3% in the patients younger than 19 years, but higher in those aged 60?69 years at 59.4% and in those older than 70 years at 83.3% (P<0.05).

Conclusion: For cases of fish bone ingestion, esophagogastroduodenoscopy should be carefully performed, especially in patients younger than 19 years. The fish bone detection rate by esophagogastroduodenoscopy was high among the elderly patients. Therefore, whether esophagogastroduodenoscopy is necessary should be determined only after endoscopy has been performed.
