Correlation between Peptic Ulcer Disease and Risk Factors

Abstract
Background: Active peptic ulcer disease has been widely reported on endoscopic screening examination. The aim of this study was to evaluate the correlation between peptic ulcer disease and risk factors.

Methods: The study evaluated the endoscopic findings of 1,971 patients who visited Namwon Medical Center from January to June 2014. The correlation between peptic ulcer and known risk factors, including the presence of Helicobacter pylori, non-steroidal anti-inflammatory drug (NSAID) use, gender, age, smoking, and drinking, was evaluated using simple and logistic regression analysis.

Results: Of 1,971 patients, gastric ulcers were present in 348 (17.7%), duodenal ulcers in 90 (4.6%), and peptic ulcers in 438 (22.3%). The H. pylori infection rate was 30.5% in the gastric ulcer group and 30.0% in the duodenal ulcer group. On regression analysis, gender showed no significant correlation with peptic ulcer. However, the ulcer rate increased with age (odds ratio [OR]=1.02; 95% confidence interval [CI], 1.01-1.03), current smoking (OR=6.92; 95% CI, 5.02-9.53), drinking (OR=3.53; 95% CI, 2.55-4.89), NSAID use (OR=4.61; 95% CI, 3.43-6.18), NSAID use combined with drinking (OR=7.57; 95% CI, 2.26-25.35), NSAID use combined with smoking (OR=12.75; 95% CI, 6.18 -26.30), and drinking combined with smoking (OR=11.25; 95% CI, 6.32- 20.02). All showed significant P-values.

Conclusion: Active peptic ulcer has been widely reported on endoscopic examination during health screening. In this study, we evaluated the correlation between peptic ulcer and risk factors. H. pylori infection, age, smoking, drinking, and NSAID use were correlated with peptic ulcer.
