Seroepidemiology of Hepatitis A in One Korean Secondary Hospital Health Center in the Last 4 Years and Vaccine Indication

Abstract
Background: The anti-hepatitis A virus (HAV) total seropositivity rates in adults aged >20 years have dropped significantly in recent years. Moreover, the prevalence of HAV infection has been rapidly increasing since 2000. We assessed the ant-HAV total seropositivity rates in adults who underwent health check-ups in the last 4 years in order to identify indications for HAV vaccination.

Methods: From January 2012 to December 2015, we performed a survey to determine the anti-HAV total seropositivity rates in adults who received health check-ups at a secondary hospital in Seoul, according to age and sex. We analyzed yearly seropositivity rates because they were expected to be low in adults born after the 1970s.

Results: The overall anti-HAV total seropositivity rate was 78.1% (7,688 of 9,838). The rate in men was 79.5% (4,146 of 5,214), significantly higher (P<0.05) than that in women (76.6%, 3,542 of 4,624) by 2.9%. Furthermore, the anti-HAV total seropositivity rate in adults born before 1960 was significantly higher (P for trend <0.05) than that in adults born in 1980?1989. The seropositivity rate in adults born after 1976 has decreased especially rapidly.

Conclusion: The anti-HAV total seropositivity rate in adults born after 1976 has decreased rapidly. Thus, the results of this survey may help to establish a vaccination policy against HAV infection in adults and children.
