Difference of Treatment Target between Direct Measurement and Calculated Measurement of Low Density Lipoprotein Cholesterol According to the Severity of Fatty Liver

Abstract
Background: Low-density lipoprotein cholesterol is a primary contributor to cardiovascular and other diseases, and elevated levels should be treated. This study aimed to compare the calculated and actual measurement methods for fatty liver, which is known to affect blood lipid levels, and to determine the effect of measurement differences on the decision to treat fatty liver disease.

Methods: Study subjects were male and female adults aged over 55 years who had been diagnosed with mild fatty liver in 2015. Family and personal medical histories were obtained with a self-administered questionnaire and an interview. Values for low-density lipoprotein cholesterol measured with the homogeneous method were compared with calculated values obtained with the Friedewald formula.

Results: There was a significant difference between the measured value and the calculated value in patients diagnosed with fatty liver. As the severity of fatty liver disease increased, the number of incorrect results also increased. In addition, there was a significant difference in the number of subjects determined to need treatment according to the measurement method.

Conclusion: In cases of fatty liver, the actual measured value rather than the calculated value is preferred for the proper selection of patients to be treated and for the evaluation of treatment adequacy.
