Relationship between Shift Work and Coronary Artery Calcium Score

Abstract

|
Background: Shift work is well known to increase the risk of coronary heart disease (CHD). However, only few studies have been conducted in South Korea regarding the relationship between shift work and CHD. This study investigated the relationship between shift work and CHD in South Korea by using coronary artery calcium score (CACS).
Methods: The study subjects were composed of 1,587 workers who visited the health promotion center at Ulsan University Hospital and underwent CACS between January 1, 2015 and June 30, 2016. We excluded patients previously diagnosed as having CHD. They were divided into two groups according to work pattern (shift work or not), and t-test and chi-square test were used to evaluate baseline characteristics between the two groups. CACS was categorized into CACS of ¡Â0 and >0. We performed multivariate logistic regression analysis to investigate associations between shift work and CACS.
Results: The CACS of the shift workers was significantly higher than that of the non-shift workers (94.27 vs. 33.34; P¡´0.05). In the shift-work group, the odds ratio (OR) for CACS of ¡µ0 was significantly higher (OR, 1.90; 95% confidence interval [CI], 1.50?2.41). This result was consistent after adjusting for confounders (age, sex, systolic blood pressure, total cholesterol, high-density lipoprotein cholesterol, hypertension, diabetes mellitus, smoking, drinking, waist circumference, physical activity, education level, and monthly income; OR, 1.64; 95% CI, 1.17?2.29).
Conclusion: Shift workers had higher CACSs than the traditional daytime workers, and the odds of high CACS in the shift workers was high after adjusting for cardiovascular risk factors. We should pay attention to modify lifestyle and educate to prevent CHDs in shift workers.
