Nutritional Status after Gastrectomy in Normal and Underweight Patients with Early Gastric Cancer

Abstract
Background: The detection and survival rates in early gastric cancer (EGC) increased in Korea. This 5-year retrospective cohort study was performed to investigate the nutritional status in normal and underweight EGC survivors after gastrectomy.

Methods: We included 233 EGC survivors (115 men and 118 women, mean age: 51.9 years) with an initial body mass index of £¼23 kg/m2 who underwent gastrectomy. We analyzed the longitudinal changes in body weight and nutritional parameters at baseline and at 1, 2, 3, 4, and 5 years after surgery.

Results: Body weight decreased during the 5-year period after gastrectomy, from 59.2 kg to 56.7 kg in men and from 51.9 kg to 49.2 kg in women (P£¼0.001). Protein, albumin, total cholesterol, and aspartate aminotransferase levels increased (P£¼0.001), although random glucose levels decreased (P£¼0.01) in both the men and women. The proportion of being underweight increased during the 5-year period from 8.7% to 22% in men, and from 5.9% to 24.4% in women (P£¼0.001). The proportion of anemia increased during the 5-year period from 13.9% to 25% in men, and from 26.3% to 43.4% in women (P£¼0.05).

Conclusion: After gastrectomy in normal and underweight EGC patients, the risks of being underweight increased by threefold in the men and by fivefold in the women, and the risks of anemia increased by twofold in both the men and women. Periodic nutritional monitoring and management are needed in normal and underweight patients after gastrectomy.
