Treatment in Patient with Severe Bilateral Ureteropelvic Junction Obstruction

Abstract
We report a case of infant with severe bilateral ureteropelvic junction obstruction. The boy presented with bilateral hydronephrosis at prenatal ultrasonography. Voiding cystourethrography revealed no reflux at the age of 3 days. Diuretic renogram showed differential renal function of right and left was 51.6% and 48.4%, respectively. Ultrasonography at 6 weeks revealed bilateral aggravation of hydronephrosis that was estimated as grade IV in both sides. Therefore, dismembered pyeloplasty was performed for the left side at the age of 2 months. Four months later, pyeloplasty for the right side was done as febrile urinary tract infection and aggravation of hydronephrosis. Ultrasonography performed at 9 months after second surgery showed left hydronephrosis had improved to grade II and right had improved grade III or IV. There was an increase of parenchymal thickness in both sides.
