Testicular tumors are very rare in children. We report here on an interesting case of an infant with immature teratoma that mimicked a scrotal hydrocele. A 6-month-old boy presented with an enlarged and transilluminating scrotal mass in the left scrotum. Ultrasonography revealed a 3x2.94 cm sized cyst, including a 5 mm sized, lobulated, round and hyperechoic mural nodule. On surgical exploration, the cystic lesion looked like hydrocele was found inside of a tunica albuginea layer and a mural nodule was laid on the inner surface of it. Radical excision was performed and the structure was proven to be of a cystic mature and immature teratoma. One should consider testicular teratoma in a child with an apparent scrotal hydrocele and who presents with unusual findings on ultrasonography such as abnormalities of the ipsilateral testis.
