Laparoscopic Marsupialization of Retrovesical Cysts in Children with Recurrent Epididymitis

Abstract
Retrovesical cysts are histologically classified into seminal vesicle cysts, prostatic utricular cysts and ejaculatory duct cysts. While asymptomatic in most cases, some cysts are associated with recurrent urinary tract infections, stones, voiding problems and even malignancies, warranting surgical treatment. This report outlines our experience and reviews the literature to evaluate the results of laparoscopic excision of retrovesical cyst. Two boys with intractable recurrent epididymitis underwent laparoscopic excision. Antibiotic prophylaxis was not helpful in relieving recurrent infection. One patient with a history of proximal hypospadias showed a tract between the cyst and urethra, whereas the other did not.
To mark the location of cyst, either transvesical indigocarmine injection or transurethral placement of light source was used depending on the presence of tract. The cyst was easily identified, dissected and unroofed by excision. To avoid peritoneal contamination, the remaining cyst wall was intracorporeally sutured. The operation time in each patient was 150 and 180 minutes, respectively. The postoperative courses were uneventful and no patients reported recurrence of epididymitis during 6 months of follow-up without any prophylactic antibiotics. Laparoscopic approach to retrovesical cyst is a safe, effective and minimally invasive surgical procedure allowing rapid recovery for the patient.
