Microscopic Hematuria in Children

Abstract
Asymptomatic microscopic hematuria is relatively common with an estimated prevalence of 0.5% to 2.0%. Detection of blood in a child¡¯s urine can be the source of immense anxiety for the patients and their parents. As such, detection of persistent microscopic hematuria prompts a thorough evaluation of causation. However, the clinical significance of asymptomatic hematuria is unclear and the merit of such an evaluation has not been confirmed. Since 1998, a mandatory urine mass screening program of elementary, junior high, and high school students has been carried out in South Korea. Simultaneously, detection of microscopic hematuria along with referrals to nephrologists and urologists have increased.
However, the evaluation of pediatric microscopic hematuria is often inconclusive, and most children with isolated cases of hematuria have undergone benign disease processes. Extensive evaluation, particularly invasive renal biopsy, should be performed on selective patients. Furthermore, regular follow-up urinalysis is recommended for identifying isolated cases of hematuria in children.
