Diagnosis and Treatment of Acute Intravaginal Spermatic Cord Torsion

Abstract
Acute scrotum is defined as ¡¯a new onset of pain, swelling, and/or tenderness of intrascrotal contents¡¯ and is a common presenting complaint in pediatric urology. Among various etiologies causing acute scrotum, acute intravaginal spermatic cord torsion is one of the most important conditions to clinicians because delay of diagnosis may result in testicular loss.
Clinical presentation of acute intravaginal spermatic cord torsion is diverse, so clinicians should be cautious with patients who complain of scrotal pain or unexplained inguinal or abdominal pain. Though color doppler ultrasonography is known as a rapid and reliable modality in diagnosing spermatic cord torsion, established sensitivity in proven cases of spermatic
cord torsion is a disappointing 63% to 90%. In this review, new promising and experimental diagnostic modalities are briefly discussed. There is no common recommendation about preferred type of fixation, many urologists currently used a Dartos pouch orchiopexy. Subfertility is found some patients after torsion, but results of future fertility vary and are conflicting. Long-term data of torsion cohorts and more experimental studies are needed to establish fertility issues.
