Persistent Mullerian Duct Syndrome with Crossed Testicular Ectopia Complicated by Dysgenetic Testis

Abstract

|
Transverse testicular ectopia is a rare congenital anomaly in which both testes migrate toward the same hemiscrotum. A 18 month-old boy was referred to the Urology department with left inguinal swelling and a right empty scrotum discovered at birth. A testis ultrasound exam revealed left inguinal hernia with a normal left testis, however the right testis was not
identified in the scrotum or inguinal region. Laparoscopy revealed a closed right inguinal ring, and a glistening band which was connected to an ovoid, white mass entering the ring. An intraoperative frozen biopsy suggested dysgenetic gonads and a possibility of remnant Fallopian tubes. We performed a total abdominal hysterectomy with bilateral gonadectomy. The final pathology of both ovoid masses was a bilateral dysgenetic testis and epididymis with bilateral Fallopian tubes attached.
