Abstract
Epididymitis is one of the most common causes of acute scrotum. Extended-spectrum beta-lactamases (ESBL) are plasmid mediated enzymes that cause resistance to various types of antibiotics. We report a case acute epididymitis caused by ESBL-producing bacteria presented with scrotal pain and fever. His symptoms and fever were not relieved in
spite of empirical parenteral antibiotics treatment. After identifying ESBL (+) E coli in urine culture, ertapenem parenteral treatment relieved his clinical symptoms and sings and he was discharged without complications.
