Purpose: We report a case of orbitofrontal cholesterol granuloma.
Case summary: A 47-year-old man complained of swelling of right upper eyelids and right proptosis for three months. The ocular examination of the patient revealed 6.5 mm of right exophthalmos and inferior displacement with upgaze limitation of the right eye. Orbital CT showed a well-defined, homogeneous mass with osteolytic lesion in the superotemporal aspect of the right orbit. Orbital MRI showed the mass with high signal intensity in both T1- and T2-weighted images. The mass was approached via lateral orbitotomy and totally removed. Histopathological examination revealed a cholesterol granuloma. And there was no sign of recurrence after 7 months follow-up.
