A Case of Myopic Progression after Oral Intake of Pilocarpine

Abstract
A 29 year old Myopic male patient took new glasses (OD; -4.00Dsph, OS; -3.75Dsph -0.5Dcyl axis 90¡Æ) for sudden decrease of visual acuity 3~4 days ago. Several days later, he felt more decrease of vision and visited our clinic. The patient¡¯s vision with glasses was 0.63 in each eye and cycloplegic refraction demonstrated refractive errors of OD; - 4.75Dsph -1.00Dcyl axis 40??, OS; -5.00 Dsph -0.75 Dcyl axis 140??and the best corrected visual acuity was 1.0. He was diagnosed as Sjo¡§gren syndrome in August 2006 and had been taking oral pilocarpine for recent one week. As myopic progression was thought to be induced by oral pilocarpine, he was ordered to discontinue the medication. The patient¡¯s vision improved to 0.8 with his own glasses after a week, and to 1.0 in each naked eye after 1 month. Thereafter, his myopic refractive errors progressively increased up to -3.25 Dsph -0.5 Dcyl axis 20??in the right eye and -3.00 Dsph -0.50 Dcyl axis 150??in the left eye with no further significant change. Oral pilocarpine may induce myopia and should be prescribed with caution.
