Extended Indication of 2-octylcyanoacrylate: Consideraion about Replacement for Cutaneous Stitch

Abstract
Purpose: Recently, the effectiveness of Dermabond? (2-OCA, 2-octylcyanoacrylate) in facial plastic surgery has
been demonstrated as alternative of cutaneous stitch, but cost component and limited indication made incomplete
alternative of cutaneous closure.

Material and Methods: 414 cases with hand trauma, scar revision, skin tumor, wound dehiscence, operation for osmidrosis, facial laceration, and distant flaps, from March 2003 to July 2008 were enrolled into this clinical study. Hematoma, infection, wound disruption were followed up during one month to eight month after operation. In sort of site, head and neck in 234, axilla in 46, trunk in 18, genitalia in 12 , buttok in 19, upper extremity in 31, and lower extremity in 54 cases were applied with 2-OCA except foldable upper lid and mucosa component.

Results: Complication after application of 2-OCA was wound disruption in 6 cases. The other cases had no complications such as hematoma, or infection in followed-up period at 1st, 7th day, 2nd week, 1st to 8th month after application. Accurate and appropriate approximation of dermal, subcutaneous, muscle fascia with nonabsorbable suturing made no disruption even in abdomen and chest.

Conclusion: 2-OCA could be applied in any site of skin incision and couldn¡¯t be expensive in thinking of less dressing rate, less traffic expenses, less clinic visit than conventional wound closure. We consider that tissue adhesive such as 2-OCA can make extensive use of application.
