One-lung Ventilation with Coopdech Endobronchial Blocker in the Paient with Incapable Mouth Opening -A Case report -

Abstract

|
Lung isolation in the thoracic surgical paitient can be achieved with the use of a double-lumen endotracheal tube, a univent tube or an independent bronchial blocker. If some patients requiring lung lisolation have a difficult airway because of facial trauma, the use of bronchoial blockers have the preference. We have an experience of successful management for one-lung ventilation with bronchial blocker in patient who was intubated in the awakened state through the nasal cavity under fiberoptic bronchoscope guidance because he could not open his mouth due to intermaxillary fixation. In this case, we used the bronchial blocker(Coopdech endobronchial blocker tube, Daiken, Japan) which has the tip with prefixed bend and the connector between endotracheal tube and ventillator, with the port for fiberoptic bronchoscope and blocker. We believe that bronchial blocker is the best choice for the patient with difficult airway like this and so report this case.
