A Case of Neonatal Giant Hepatic Hemangioendothelioma Associated with Kasabach-Merritt Syndrome - A Case Report -

Abstract
We experienced a case of neonatal giant hepatic hemangioendothelioma. The abdominal mass was detected by fetal echogram at 35 weeks of gestational age in this patient. The patient was delivered by Caesarean section at 38 weeks and 3 days of gestational age. Abdominal distension due to the giant abdominal mass was notified. SpO2 was not maintained over 90% without administration of oxygen 5L via hood after birth because of respiratory distress due to the giant abdominal mass. Blood analysis revealed anemia, thrombocytopenia and prolonged coagulation time, which indicated disseminated intravascular coagulation. (DIC). Ultrasonography revealed a 7cm-sized hepatic mass in segment 5, with multiseptated cystic nature and abundant vascular flow supplying from ductus venosus. Under a diagnosis of hepatic hemangioma with Kasabach-Merritt syndrome, respiratory distress was aggravated as time goes on, so we performed a surgical resection on the first day of life, and then the laboratory findings of DIC was subsided. The histologic finding was proved as hemangioendothelioma. The patient was discharged home 2 weeks after the operation. It is likely that surgical resection was an effective and safe treatment for neonatal giant hemangioendothelioma when complicated with a condition such as respiratory disorder and DIC.
