Bilateral Central Retinal Vein Occlusion due to Hyperviscosity Syndrome in Multiple Myeloma

Abstract
Purpose: To report a case of bilateral central retinal vein occlusion associated with multiple myeloma.

Case summary: A 73-year-old woman came to our clinic because of sudden loss of vision in both eyes for 2 days. Best-corrected visual acuity was 0.16 in the right eye and 0.08 in the left eye. She was diagnosed with bilateral central retinal vein occlusion by fundoscopic examination and fluorescein angiography. Further medical workup revealed that the cause of her retinal findings was a hyperviscosity syndrome secondary to multiple myeloma. After an intravitreal injection of bevacizumab in both eyes, visual acuity was improved to 0.5 in the right eye and 0.32 in the left eye.

Conclusion: Although ophthalmic manifestations are rare, they can still be the first signs of multiple myeloma. Therefore, in patients with bilateral central retinal vein occlusion in the absence of diabetes or hypertension, a comprehensive medical workup should be performed to search for the presence of other systemic diseases such as multiple myeloma.
