Development of Spinal Cord Edema in the White Matter after Anterior Decompression for Cervical Myelopathy - A Case Report -

Abstract
A 56-year-old man presented with numbness of both hands and quadriparesis with bilateral extensor plantar responses. Radiologic studies revealed diffuse bulging with severe cord compression and stenosis at C5-6. The patient underwent C5 corpectomy and anterior cervical interbody fusion using titanium mesh cage(Medtronic sofamor Danek, Memphis, TN) at C4~6. Two weeks after surgery T2-weighted MR imaging at the C5-6 level revealed high signal intensity in the white matter of the spinal cord. T2-weighted MR image obtained 2 months after surgery demonstrated a significant decrease in an area of high signal intensity and the patient achieved good recovery of motor function. We present a case of spinal cord edema after decompressive surgery for cervical myelopathy. The unusual presentation after operation may result from disturbed local venous circulation.
