Unexpected airway anomaly is a life threatening event during induction of general anesthesia. We report a case of difficult intubation where the laryngeal inlet was obscured by epiglottic hematoma. The patient have been operated due to right nasoethmoidal and inferior orbital surface fracture. During induction of reoperation for bleeding control 5 days after previous operation, epiglottic hematoma was detected. Direct laryngoscopic intubation was failed 3 times because of difficult finding of valecula and epiglottis. However, mask ventilation was possible, fortunately, and a we could perform nasal fiberoptic bronchoscope-aided intubation. He was discharged 7 days later with no harmful events.
