Diabetes Mellitus and Insulin Resistance in Rheumatoid Arthritis

Abstract
Rheumatoid arthritis (RA) is a systemic autoimmune disease that characterized by a chronic synovial inflammation
leading to cartilage and bone destruction. Although the pathogenesis of RA is not clearly determined, the number of
macrophages and proinflammatory cytokines such as tumor necrosis factor-¥á (TNF-¥á), interleukin (IL)-1, IL-6 might
play a role in the mechanism of chronic inflammation in RA. Some potent evidences that immune regulatory mediators
including TNF-¥á, IL-1, and IL-6 shared common pathogenic pathway in RA, insulin resistance (IR), and diabetes
mellitus (DM) have been identified. However, the complex interaction among cytokines and other mediators involved
in these chronic inflammatory diseases are not fully elucidated. In this article, author describes the effects of pathogenic mediators for inflammation and anti-rheumatic drugs including traditional disease-modifying antirheumatic drugs, corticosteroids, TNF-¥áantagonists on glucose metabolism or metabolism abnormalities such as IR and DM.
