Abstract
We report a case of acute kidney injury associated with acute non-fulminant hepatitis A, for which kidney biopsy
revealed acute interstitial nephritis with no evidence of glomerular involvement. A 28-year-old man was admitted to
our hospital because of fever and azotemia for 7 days. Initial blood biochemistry showed blood urea nitrogen 66.4
mg/dL, creatinine 13.4 mg/dL, aspartate aminotransferase 218 IU/L, alanine aminotransferase 491 IU/L, and total
bilirubin 13.3 mg/dL. Serologic tests were positive for anti-HAV IgM and negative for HBsAg, anti-HBc IgM, and
anti-HCV Ab. He was treated with combination therapy including intravenous diuretics, albumin, and hepatotherapy.
In view of progressive azotemia with oliguria, renal biopsy was performed and hemodialysis was initiated. He
gradually improved with no further uremic symptoms after a total of 12 days of dialytic support including
hemodialysis and continuous hemodiafiltration. Thereafter, he was discharged with normalized condition.
Considering the result of this case, kidney biopsy and early treatment with dialysis should be performed in a patient
with oliguric acute kidney injury associated with non-fulminant acute hepatitis A.
