Diagnosis and Treatment of Gastric Dysaccommodation in Functional Dyspepsia

Abstract
Gastric accommodation response is an important mechanism of normal gastric physiology, which provides a storage
capacity for the meal. Several factors may contribute to initiating and maintaining gastric accommodation in response
to meal ingestion. Duodenal presence of nutrients seems to be the relatively strongest trigger for the gastric
accommodation reflex. Abnormal gastric accommodation is present in 40% of patients with functional dyspepsia and
little is known about the etiology. Currently available diagnostic tests that evaluate gastric accommodation pose
technical difficulties and clinical impact remains controversial. The clinical evidence supporting role of gastric
accommodation as an important therapeutic target is currently lacking. Recently studied drugs including buspirone,
tandospirone and acotiamide provide improvement of gastric accommodation and postprandial symptoms in
functional dyspepsia patients. Further studies investigating efficacy and safety of these drugs are awaited.
