Abstract

Laparoscopic procedures are considered relatively safe and noninvasive. However, there exist small but serious
complications related to insufflations with carbon dioxide (CO2) gas. These include hypercapnia, subcutaneous
emphysema, pneumothorax, and pneumomediastinum. Surgeons and anesthesiologists must be aware of these
complications and what types of patients are at a higher risk of developing them. We present a case of the delayed
detection of the endobronchial intubation in a patient undergoing laparoscopic nephrectomy. In this case, Bispectral
index (BIS) was severely decreased, and severe hypercapnia is highly suspected to be the cause of low BIS.
