There are two bony processes in talus, posterior and lateral, both of which are subjected to fracture. The posterior process of the talus is composed of medial and lateral tubercles separated by a groove for the tendon of the flexor hallucis
longus. The
lateral tubercle is the longer of the two and projects more posteriorly.
In the fracture of the talar process, many cases of the fracture of the lateral process has been reported. But, the fracture of the posterior process, especially entire, is extremely rare and has not been reported except one case.
Open reduction and internal fixation is mandatory in such a fracture because the undersurface of the posterior process is composed of articular cartilage, forming 25% of the posterior articular facet of the subtalar joint.
Recently, we experienced a case of fracture of the entire posterior process of the talus and we will present this rare injury in detail.
