Nasopharyngeal Carrier of Staphylococcus aureus among Healthy Individuals and the Antimicrobial Susceptibility of Methicillin-Resistant Staphylococcus aureus

Abstract
Methicillin-resistant, Staphylococcus aureus (MRSA). is extremely important that it be quickly and accurately recognized for proper selection of antimicrobial agents for therapy and for hospital infection control. It commonly used disk diffusion antimicrobial susceptibility test is prone .to give both false methicillin resistance and susceptibility. In this study, strains of S aureus, 82 from healthy students of Wonkwang health junior college, and 45 from Wonkwang university hospital, which were isolated during the period from Aprail to May, 1994, and from May to June, 1995, were tested for their drug resistance was investigated. The method of MRSA detection, methicilin susceptibilities by the agar dilution, agar screening and disk diffusion tests, were also compared.
This studies showed the following.
1. S aureus was isolated from 82% of 546 persons (15%), while that were 11% in male and 16% in female. No difference was noted in the prevalence rate by 1994 and 1995
2. By the agar dilution test, 8% and 73% of S. aureus from healthy students and patients of Wonkwang university hospital respectively were methicillin resistant.
3. None of the MRSA was found to be heteroresistant. MRSA detection by the agar dilution and by the disk diffusion test respectively were 98.2% and 94.6% agreement with agar screening test, while that were 100% by he agar dilution and agar screening test.
4. The minimum inhibitory consentration (MIC) against 37 strains of MRSA were ranged from 16 to 128 pg/ml, MIC0 and MICso >128 gg/ml in 1994 and 1995. On the other, the MIC of methicillin against 27 isolates of methicillin-susceptible Staphylococcus aureus ( MSSA) were ranged from 1 to 8 gg/ml , MICo 2 gg/ml, MICgo 4 pg/ml. A 32 fold difference betwween MIC of methicillin against MRSA and MSSA was noted.
5. More than 73% of MRSA strains were resistant to erythromycin, gentamicin, penicillin G and tetracycline, and more than 45% to cephalothin and clindamycin by 1994, and more than 85% of MRSA isolates were resistant to cephalothin, erythromycin,
gentamicin, penicillin G and tetracycline, and more than 68% to clindamycin by 1995.
It is concluded that healthy individuals sometimes.. carry MRSA in their anterior nares, and MRSA is quite prevalent at hospital. MIC of methicillin against the majority of MRSA is high and heteroresistant MRSA is rare. Agar screening test is practical method and an accurate for the detection of MRSA.
