A Study of Refined Diagnosis Related Groups

Abstract
The aim of this study is to guide a Refined Diagnosis Related Groups.
Diagnosis Related Groups are being used throughout the health care industry to address issues of cost, effectiveness, quality of care, and the prospective reimbursement of patient hospital days. But the DRGs do not adequately adjust for patient severity and that under a DRG-based prospective payment system. So the Refined DRGs are a major step in addressing the limitations of current DRG structure. The general structure of the RDRG system for all patients except neonate is summarized below.
1. Patients are first assigned to one of the Major Diagnostic Categories.
2. Within an MDC, patient with a tracheotomy are then isolated .
3. The remaining patients are categorized as surgical or medical based on the presence or absence of an operating room procedure
4. Surgical patients are then subdivided by type as procedure.
5. Medical patients who die within two days of admission form a separate group, early Death.
6. Medical patients are then categorized based on principal diagnosis.
For the surgical CRGs four refinement classes have been established: minor or no substantial CCs, moderate CCs, major CCs, and catastrophic CCs. These classes number from 0 to 3.
For the medical CRGs three refinement classes have been established minor or no substantial CCs, moderate CCs, and major CCs. These classes are numbered from 0 to 2.
