Left Atrial Enlargement. An Echocardiographic Assessment of Electrocardiographic Criteria

Abstract

|
A comparison of electrocardiographic manifestations of left atrial enlargement (LAE) and left atrial size by echocardiography was made in 121 patients with sinus rhythm. Electrocardiographic criteria for LAE were L; P wave duration in lead II>0.12 sec., Va; the ratio of the duration of negative terminal P in V1 to PR segment >1.0, Vb; negative P terminal force in Vl<-0.03mx sec.. The echocardiographic criteria for the diagnosis of LAE used were end-systolic left atrial dimension (>4cm) and the ratio of left atrial dimension to aortic root dimension(> 1.1.7) .
The overall sensitivity of the electrocardiographic criteria was 48%, the accuracy 61%, the specificity 84% and the predictive index for LAE 84%.
Changes in P wave morphology may be used as a reasonably specific but less sensitive indicator of LAE especially in cases with mild LA enlargements.
