Disturbance of the Coagulatory System in Patients with Acute Head Injury



Abstract
Through the study of the blood coagulator disturbance in patients with acute head injury, the relationship between the coagulation abnormality and prognosis was evaluated. Forty cases (male 30, female 10) were classified into four groups according to brain CT findings: 26 cases of cerebral contusion with acute subdural hematoma, 4 cases of acute epidural hematoma, 5 cases of skull fracture and 5 cases of cerebral concussion. In order to find the coagulatory abnormality, blood platelet count, PT, APTT, fibrinogen and serum FDP were selectively checked. The results obtained were as follows:
1) Incidence of disseminated intravascular coagulation (DIC) following head injury in 40 cases was 25% (10 cases), of which 7 cases were died and 3 cases were survived.
2) In 7 dead-cases, severe coagulopathy was observed.
3) In intracerebral injury group, serum FDP was increased.
4) The elongation of the coagulation time (especially APTT) and increase of the FDP were the most important findings to predict the prognosis of head injury.
These results suggest the evaluation of the coagulatory abnormality in patients with acute head injury is not only important to estimate the occurrence of disseminated intravascular coagulation of head injury but also useful to predict
the severity and prognosis of head injury.
