A Clinical Observation on 239 Cases of Typhoid Fever



Abstract
A clinical observation was done on 239 cases of typhoid fever admitted to Jeonbug National University Hospital from March 1975 to November 1981. The following results were obtained.
1. Both sexes were attacked almost equally. The most prevalent ages were between 21 to 30 years amounting to 37.7% of total cases.
2. Seasonal occurrence was prominent in June, July and August. In these months 42.3% of total cases was noted. Difference of seasonal occurrence was not significant.
3. Clinical symptoms on admission were fever (80, 3 %) , chill (62.7 %) , headache (57.4 %) , generalized ache(55.2%), anore2ia(25.1%), diarrhea(17,6%) and abdominal pain(15.1%) in order of frequency. The most frequent phsical finding was ileocecal tenderness which
was 27.6% of total cases.
4. Peripheral blood examination showed leukopenia in 55 cases (23.0 %) , leukocytosis in 40 cases(16.7%) and anemia in 18 cases(7.5%).
5. Widal reaction(over 1 :160) was positive in 1.66 cases(69.5%). Bacterial cultures performed on admission revealed positive blood cultures in 13.1 % and positive stool cultures in 10.3%.
6. The average duration for defervescence in patients treated with chloramphenicol and/or ampicillin (3.Ogm/day, 4.06.0gm/day respectively) was 3.8 days.
7. The complications of typhoid fever were intestinal perforation (4 case ; 1.7 %) , intestinal hemorrhage (3 cases ; 1.3 % ) , hepatitis (4 cases ; 1, 7 % ) and meningitis (2 cases ; 0.8 % ).
