Operative Treatments and Prognostic Factors of Monteggia Fracture in Adults

Abstract
Background and Obiectives : This study was performed to evaluate the clinical results of operative teatments, complications and prognostic factors of Monteggia fracture in adults.

Materials and Methods : We reviewed 53 cases of Monteggia fracture who were treated operatively from January 1985 to December 2000. We analyzed retrospectively the data according to the age, gender, causes of the injury, classification by Bado, methods of treatment and complications.

Results : Posmiddleerative complications were synostosis in 5 cases, distal radioulnar joint subluxation in 3, delayed union in 3 and nonunion in 3 cases. According to the Bruce¡¯s criteria, outcome was excellent in 19(36%) cases, good in 16(30%), fair in 10(19%) and poor in 8(15%).

Conclusion : For a good result, it is necessary to get the early diagnosis, early reduction of dislocated radial head and rigid fixation of fractured ulna. We can suppose poor results when diagnosis or operation was delayed, ulnar fracture was severely comminuted or involved ulnotrochlear joint, open fracture, and another fracture combined in the ipsilateral forearm.
