Hemodynamic Effects of Intermittent Positive Pressure Ventilation and High-Frequency Jet Ventilation with Positive End-Expiratory Pressure in Lung-Lavaged Dogs


Abstract
High-frequency jet ventilation(HFJV) with positive end-expiratory pressure (PEEP) was compared to intermittent positive pressure ventilation (IPPV) PEEP on hemodynamic effects in a model of adult respiratory distress syndrome (ARDS0 by serial
lung
lavage with prewarmed physiological saline in 10 Korean mongrel dogs. One group of subjects applying for HFJV (n=5) was ventilated with room air, driving pressure 1.055kg/§², ventilatory freqnency 90-100 breath/min and inspiratory to expiratory
time(I :
E) ratio 1 : 1. The other group of subjects applying for IPPV (n=5) was ventilated with room air, tidal volume 10 ml/kg, ventilatory frequency 20 breath/min and I : E ratio 1 : 1. Thereafter, 5, 10, 16 and 20 cmH2O PEEP were applied for 20
minutes
to
all minutes to all subjects of two groups, respectively. Hemodynamic variables such as sBP, dBP, MAP, HR, CVP, MPAP, PCWP, and CO were evaluated at each PEEP level. In addition, SVR and PVR were calculated.
@ES The results were as follows:
@EN 1. SBP was decreased at 5, 10 and 15 cmH2O PEEP level during HFJV/PEEP. DBP was also decreased at 15 cmH2O PEEP level during HFJV/PEEP and CPPV while MAP was decreased at 15 cmH2O PEEP level during HJFJV-PEEP and at 5, 10 and 15 cmH2O PEEP
level
during CPPV.
2. CVP was increased at 10, 15 and 20 cmH2O PEEP level during both ventilation where ase PCWP was increased at 20 cmH2O PEEP level during both ventilation, MPAP was increased at 15 and 20 cmH2O PEEP level during CPPV and at each PEEP level
during
HFJV-PEEP.
3. CO was decreased at each PEEP level during both ventilation.
4. SVR and PVR were increased at 10, 15 and 20 cmHoO PEEP level during CPPV and the each PEEP level during HFJV/PEEP.
In summary, the hemodynamic effects of HFJV/PEEP and CPPV were similar in accordance with the increment of PEEP level in a model of ARDS. It can be suggested that application of HFJV/PEEP to ARDS is effective like as CPPV.
