Studies on the Sclerosis of the Temporal Bone in Chronic Otitis Media


Abstract
Whether chronic otitis media can result in sclerosis of the temporal bone pneumatization has been a subject of considerable debate. The author has already discussed in the preceding article the sclerosis of the temporal bone pneumatization
roentgenologically, In this paper the author has attempted histopathological studies based on the surgical specimens of mastoid cells obtained from 38 cases of radical mastoidectomy,
Following observations are made
1) Roentgenologically all the 38 cases revealed sufficient sclerosis of the temporal bone to suggest a highly underdeveloped pneumatization,
2) The mucosal epithelium of the mastoid cells consisted largely of cuboidal cells with a few areas of simple columnar cells in some cases. Where there was a marked inflammatory reaction, areas of squamous epithelium were also found. In general the epithelium was either partially destroyed or partially desquamated and the mucous membrane as a whole presented infiltration of chronic inflammatory cells, edema and fibrous change.
3) The mucosal changes are usually accompanied by a chronic inflammatory reaction of the bony surface as evidenced by round cell infiltration, increased fibrosis and an increase in number of osteoclasts and osteoblasts. In some Instances active new bone formation was seen and is thought to be due to the presence of chronic inflammation.
4) In conclusion it can be said that chronic otitis media can result of the temporal bone in the sclerosis pneumatization.
