A Case of Multiple Menigioma



Abstract
The purpose of this paper is to present a case of multiple myeloma which was experienced at Inchon Christian Hospital.
The multiple myeloma is rather rare case in Korea although a few cases were reported previously. Confirmation of diagnosis was made by an insidious onset of localized or generalized bone pain with
j various hematochemical abnormalities and repeated Xray findings. However, bone marrow aspiration
i or biopsy of the lesion and the demonstration of the abnormal plasma cells made possible to confirm final diagnosis. There is a probability of fact that a considerable number of cases are failed to reach to the well
established medical facilities where various and accurate diagnostic procedures are available. A Case Report;
The patient is 44 year "old man a taylor who has had an insidious onset of chest pain with unexplained weight loss for about 2 years duration.
The pain is localized on to left chest posteriorly along the left 7th rib under and mesial border of left scapula. The pain also was radiating to both shoulders and has been persistent and getting worse progressively.
Physical examination revealed well developed
poorly nourished man appeared to be moderately anemic but no acute distress was noted. P 78/min. R 20/min. Tem. .36 C, and BP 110/70 were in normal limits. There were no local tenderness, no swelling and no positive neurological findings.
Xray examination no admission revealed that an osteolytic lesion on left 7th. rib posteriorly with
j pathological fracture and generalized osteoporosis. A few punched-out lesions are noted on both scapulas
as well as on right 3rd, 9th and 10th rib by repeated X-ray examination of bone survey. ~i
The laboratory tests including appearance of plasma cells in peripheral blood, greatly elevated erythrocyte sedimentation rates, hyperproteinemia with reversed A/G ratio, sharp peak of gamma globulin M
in paper electrophoresis of serum protein and Bence Jones protein in the urine were characteristic.
w Finally the demonstration of characteristic malignant transformation of plasma cells to myeloma cells by bone marrow aspiration and biopsy from involved lesion of left.7th rib were made possible to confirm diagnosis.
A review of the literatures in regarding to incidences, clinico-pathological findings, prognosis and treatment on multiple myeloma is made.
