Clinical Effect of Deflazacort in Children with Nephrotic Syndrome and Juvenile Chronic Arthritis




Abstract
Twenty children patients requiring corticosteroid therapy for active chronic inflammatory disorders were entered into the study.
In 8 patients(3 men, 5 women, aged 6-15 years) a diagnosis of juvenile rheumatoid arthritis(JRA) had previously received NSAID including salicylate, but did not show good response. In 12(9 men, 3 women, aged 2-11 years), nephrotic syndrome requiring steroid therapy had been diagnosed.
All patients received oral treatment with deflazacort and the results were as follows;
1. In patients with juvenile rheumatoid arthritis (JRA) treated with deflazacort(1.2 mg/kg), 6 in 8 cases(75%) showed good response and 2 cases showed fair response.
2. In patient with nephrotic syndrome treated with deflazacort(2.4 mg/kg), 11 in 12 cases (91.7%) showed good response and 1 case showed fair response.
3. No adverse effect on laboratory test were observed, but in 1 patient with nephrotic syndrome, mild moon face and weight gain were observed, but disappeared by reducing the dose- In 2 patients with juvenile rheumatoid arthritis, who measured bone mineral density, no loss of bone mineral density observed.
These observation suggest that deflazacort may be substitutive for prednisolone for longer period of time in patients with variable diseases requiring glucocorticoid treatment.
