Three Cases of Latent Autoimmune Diabetes in Youth among Korean Obese Adoles

Abstract
Latent autoimmune diabetes in youth (LADY) has been proposed in young individuals with type 2 diabetes mellitus (T2DM) who present with beta cell autoantibodies, similar to Latent autoimmune diabetes in adults (LADA). LADA shows a greater risk of complications in the later course of the disease than T2DM. When we see LADY and LADA in an age-related continuous spectrum, LADY has more adverse results than does T2DM. The incidence of obesity and T2DM has increased remarkably in children and adolescents in Korea. High autoimmunity with beta-cell autoantibodies may be observed in these individuals. As being overweight is one of the major risk factors for LADA, the number of individuals with it is increasing. Herein, we report three cases of LADY and their clinical characteristics in detail. All the patients were obese, with a mean body mass index Z-score of 3.5. The patients are currently undergoing monitoring and lifestyle interventions without insulin administration. As more cases of LADY are expected, more effort is needed for the early detection of autoimmune markers associated with the risk of diabetes progression in adolescents.
