Blockade of NMDA Receptor in the Knee Joint Suppresses Both Pain-Related Behavior and Spinal Fos Expression Following Intra-articular Injection of Carrageenan in the Rat

Recent studies have suggested that peripheral excitatory amino acid receptors are involved in the generation of inflammatory pain. In this study, employing a rat model of arthritic pain produced by an intra-articular injection of carrageenan, we examined whether NMDA receptor in the knee joint played a role in the generation of the arthritic pain-related behavior (i.e., decrease in weight load of the affected leg) and the accompanying Fos expression in the spinal dorsal horn. Injection of the uncompetitive NMDA receptor antagonist ketamine (0.2 mg or 1 mg in 40¥ìl saline) or memantine (0.1 mg, 0.2 mg, 1 mg in 40¥ìl saline) into the knee joint, but not abdominal, cavity immediately prior to the carrageenan injection (2%, 40¥ìl) significantly inhibited the pain-related behavior. The intra-articular injection of ketamine (1 mg) or memantine (0.2 mg) also suppressed the Fos expression in the laminae I-II and laminae V-VI at the L3-4 spinal level. Subsequent statistical analyses revealed that the degree of the spinal Fos expression was strongly correlated with the extent of the pain-related behavior. These results suggest that NMDA receptors in the knee joint contribute to the generation of arthritic pain through Fos protein expression in spinal neurons.
