Related Factors of In-Home Service Utilization in Korean Long-term Care Insurance among Certificated Elderly Living Alone

Abstract
This study is to analyse related factors of in-home service utilization in Korean Long-term Care Insurance among certificated elderly living alone. Subjects of analysis were 27,004 certificated elderly living alone who were 65 years and over at the time of Jan. 1, 2010. According to the analysis, 29.2% of the subjects utilized in-home services of long term care insurance, and 27.2% of them home-visit care service and 17.9% the other in-home services, and the rates of overall in-home services were increasing among the eligibilities of health insurance and the elderly with family caregivers. Among the health characteristics, dementia, stroke, and behavioral problems, ADL of 20 points or more, the joint construction of 7 points or more, were significant variables to decrease the utilization rates of in-home services, however the factors of 1 or more of the chronic diseases, IADL of 23 points or more and upper and lower motor impairment of 6 points or more makes the utilization rates increasing. The rate of home-visit care service utilization were similar to the overall in-home services utilization. The utilization rates of in-home services except home-visit care were decreasing among the recipients who get financial help from government, and increased among the rural elderly living alone. The utilization rate of the in-home service except home-visit care were decreased among the elderly with dementia, class 3, behavioral problems, ADL of 23 points or more, the joint construction of 9 points or more, while increased among the elderly with 3 or more of the chronic diseases, IADL of 23 points or more and upper and lower motor impairment of 6 points or more. In conclusion, among the certificated elderly living alone, the utilization rate of in-home service were related with their health characteristics and the other general characteristics. And these findings may be helpful for the establishment of long term care policy for the elderly living alone through the coming aged society.
