We report a case with insulinoma diagnosed by a preoperative selective intra-arterial calcium stimulation test. A 49-year-old woman visited our hospital due to recurrent dizziness, fatigue and lightheadedness. Hyperinsulinemic hypoglycemia was suspected, but imaging studies including pancreatic magnetic resonance imaging and endoscopic ultrasonography failed to detect a pancreatic mass. We performed a selective intra-arterial calcium stimulation test. The insulin level in the patient¡¯s hepatic vein increased when calcium was injected into the superior mesenteric artery, suggesting the presence of an insulinoma in the pancreatic body. After enucleation of the tumor, the patients symptoms of hypoglycemia improved.
