Statin Therapy and Risk of Developing Type 2 Diabetes

Abstract
Although the results of previous randomised statin trials indicate that interventions that lower LDL cholesterol concentrations can significantly reduce the incidence of coronary heart disease (CHD) and other major vascular events, the relationship between LDL and the development of diabetes remains controversial. The West of Scotland Coronary Prevention Study (WOSCOPS) evaluated this relationship and reported a small protective effect associated with lowering LDL concentrations, but did not use standardised criteria for diabetes diagnoses. In the JUPITER trial, 17,802 adults without clinical or biochemical diagnoses of diabetes based on fasting glucose concentrations were assigned either rosuvastatin or placebo for a median of 1.9 years. Significantly more individuals in the statin group than in the placebo group developed diabetes. Recently, Rajpathak (YEAR) and Naveed (YEAR) meta-analysed statin and develop diabetes. Statin therapy is associated with a slightly increased risk of diabetes, but the risk is low both in absolute terms and when compared to the reduction in coronary events. Current clinical practices for patients with moderate or high cardiovascular risk or existing cardiovascular disease should not change.
