Postinjectin Soreness in Myofascial Pain Syndrome: Its Course, Associated Factors and the Effect of Acetaminophen

Abstract
Objective: To demonstrate incidence, time course, and influencing factors of postinjection soreness and to assess the effect of acetaminophen to prevent postinjection soreness.

Materials and Methods: Patients with myofascial pain syndrome of shoulder girdle muscles were administered trigger point injection, randomly assigned into one of two groups, and prescribed either acetaminophen or placebo. Visual analog scale (VAS) and Disability of Shoulder, Arm, and Hand (DASH) at baseline and 3 and 7 days after injection were measured by a blinded independent evaluator. The time of maximal postinjection soreness and of its improvement were also recorded.

Results: Twenty- nine patients (8 males, 21 females) were included. Seventeen patients (58.6%) experienced postinjection soreness. The mean onset of pain was 6 hours after the injection, which was most severe at 22 hours and started to improve at 47 hours after injection. The pain was more severe in females than in males. Especially females with chronic (£¾3 months) pain showed significantly more soreness (p=.023). Presence of postinjection soreness did not make any differences in the improvements of VAS and DASH after 7 days. Stratified analysis by sex and onset of pain revealed no significant difference in the incidence and severity of postinjection soreness between acetaminophen and placebo medication.

Conclusion: The incidence of postinjection soreness after trigger point injection was 57.6%. The soreness began at 6 hours after injection and reduced at 57 hours after injection. Significant associated factors were females and chronicity (£¾3 months) of the disease. Acetaminophen had no effect on relieving postinjection soreness.
