Tarsal tunnel syndrome (TTS) is caused by compression of posterior tibial nerve in Tarsal tunnel. The major causes of narrowing of Tarsal Tunnel under flexor retinaculum are trauma, arthritis, and diabetes mellitus. Rarely, TTS can also be induced by ganglion cysts involving posterior tibial nerve. We experienced a case of TTS in a patient who is an amateur marathoner, which might have been developed by repetitive irritation of talocalcaneal joint. A 42-year- old male complained paresthesia in right 3rd, 4th, and 5th toe. He had no specific abnormality on physical examination except Tinel¡¯s sign at right tarsal tunnel. Swellings which were not palpable were detected by ultrasonography. A ganglion that was directly compressing right posterior tibial nerve was found in right tarsal tunnel level in MRI. During surgery, these cysts were successfully removed without damage to nerve fiber.
