The patient of this case is a 77 years old female who spontaneously had muscle weakness and hypoesthesia in both lower extremities with chronic abdominal pain. From the patient¡¯s MRI, spinal epidural abscess of 1st to 3rd lumbar spine was pressing the spinal cord, and right iliopsoas muscle abscess was also found. Patient was immediately treated with broad spectrum antibiotics. She had spinal epidural abscess removal operation after 5 days of admission and maintained antibiotics treatment for 10 weeks. And she is now having rehabilitation for muscle weakness of lower extremities caused by cauda equina injury due to spinal epidural abscess. In elderly patients, clinicians should consider the possibility of occurrence of spinal epidural abscess despite absence of any specific predisposing factors.
