Controversial Aspects of the Surgical Technique in Laparoscopic Sleeve Gastrectomy

Abstract
Laparoscopic sleeve gastrectomy is newly accepted as a safe and effective stand-alone bariatric surgery. However there are so far many technical controversies regarding bougie size, staple line reinforcement, distance from pylorus where the first staple firing, section near GE junction, and the use of intraoperative leakage test. Although the procedure is technically simple, its complications such as staple line leak can be very serious and difficult to treat, even life threatening. There is an inverse relationship between effectiveness of procedure and complication rate. Lack of technical standardization should be resulted in poor outcomes. Standardization of surgical technique is indispensable for improving effectiveness and minimalizing complication rate.
