Granuloma Developed in the Side Hole of ITDAS Catheter

Abstract
Implantation of intrathecal drug administration system (ITDAS) is on the increase in cancer patients and chronic intractable pain patients to reduce the side effects of systemic opioid administration. However, the chances of complication occurring is also increasing with the frequency of implantation. We experienced a case of ITDAS catheter obstruction due to granuloma formation which is relatively common complication of ITDAS. A 61-year-old man with failed back surgery syndrome was implanted with ITDAS to control his unrelieved pain. After the successful implantation, ITDAS has been refilled with morphine almost every 3 months. When the patient visited our hospital for refill 43 months after implantation, it was found that morphine had been infused significantly less than expected. The cause of infusion failure was considered as the catheter obstruction due to granuloma formation. We confirmed that side hole and lumen of the catheter was obstructed with granuloma during the catheter change.
